we wasn't allowed to go to the toilet when I was in school - the answer would be no

We just used to walk out anyway. We might have had a 'toilet prob'  that the teacher didn't know about

No, I just used to do it on the chair, next to the fire and the crate of hot milk 

We were supposed to go at playtime/lunchtime, so it was frowned upon if you asked to go during lessons, especially if you asked quite soon after a break.
