Nikki is speaking to Ulrika about Anorexia.
I think it likely she is concerned about Ulrika's extreme weight loss as earlier she was telling Ulrika that she (Ulrika) was thinner than she (Nikki) was.

Considering the tv adds around 10lbs I can only imagine what she looks like in RL. Her clothes hang on her - doesn't look very healthy to me at all.

I've just been watching today's (Sunday's) BBLB which I'd recorded, and John McCririck was saying that he was genuinely concerned about her health after losing so much weight.
