I know you can get legionaires disease from not cleaning it,

Bacteria can grow in shower-heads where the fitting is made from a material that can be used as a nutrient source. When the shower is turned on, a spray of contaminated droplets is created. All shower fittings, whether used regularly or not, should be run for 5 minutes each week, passing hot water for the first 2 minutes

I'm not sure, but 'toid and you have reminded me.

I soak mine from time to time in limescale solution .... seems to do the trick

Works well for kettles & irons too
