Most men have a left testicle which is larger than the right (occasionally it's the reverse.)
Apart from monorchism and undescended testicles, there are numerous other conditions which affect the testicles. Some of these are based on glandular function.
There is also a condition (whose name I have forgotten) the effect of which is for the testes not to appear completely separate. This can give the appearance of a third testicle, but can often look like one large testicle.
